Image-Guided PRP for Sacroiliac Joint Pain

Evidence-based, fluoroscopically guided platelet-rich plasma injection for chronic sacroiliac joint pain — a regenerative option for patients seeking durable improvement beyond repeated steroid injections.

BASICS

What is the sacroiliac joint?

The sacroiliac (SI) joint is where the sacrum — the triangular bone at the base of your spine — meets the pelvis on each side. It is a small but heavily loaded joint that transfers force between your spine and your legs every time you walk, stand, climb stairs, or shift weight.

When this joint becomes painful, the symptoms are often misattributed. SI joint pain can radiate into the lower back, the buttock, the groin, the back of the thigh, and sometimes down the leg, mimicking sciatica. Many patients are evaluated for disc problems, hip arthritis, or general "low back pain" before the SI joint is identified as the actual source.

Common causes include pregnancy-related changes, prior trauma, lumbar fusion that shifts mechanical load, gait abnormalities, and degenerative changes that accumulate over years.

MECHANISM

How does PRP work for SI joint pain?

Platelet-rich plasma is prepared from a small sample of your own blood, processed in a centrifuge to concentrate platelets and their growth factors. When delivered into and around the sacroiliac joint under image guidance, these growth factors are thought to support tissue repair in the joint capsule, the surrounding ligaments, and the dorsal interosseous ligament complex that stabilizes the joint.

The clinical goal differs from corticosteroid injection. Steroid is anti-inflammatory and provides short-term pain relief. PRP is intended to support biological repair of the involved structures, with the aim of more durable improvement over months rather than weeks.

PRP is not an immediate pain reliever. Response is gradual and develops over six to twelve weeks.

EVIDENCE

What does the research say?

Compared to PRP for knee osteoarthritis or tennis elbow, the SI joint literature is younger but growing. Published case series and several small randomized trials have shown meaningful improvement in pain and function with intra-articular and peri-articular PRP, particularly in patients who had previously been treated with steroid injection.

The general findings:

  • Modest to moderate improvement in pain scores at 3, 6, and 12 months

  • Functional improvement comparable or superior to corticosteroid in available studies

  • Generally favorable safety profile

  • A subset of patients require more than one injection to achieve optimal response

Honest framing: PRP for SI joint pain is best characterized as an emerging, evidence-supported option — not yet a first-line treatment, but a reasonable choice for the right patient before considering more invasive procedures such as fusion.

CANDIDACY

Who is a good candidate?

PRP for SI joint pain may be considered when:

  • The SI joint has been confirmed as the pain source, ideally with a diagnostic block

  • Pain has persisted for more than three months

  • Conservative measures have been adequately tried, including physical therapy focused on pelvic stabilization, activity modification, and appropriate medication

  • You have had one or more corticosteroid injections with limited or short-lived benefit

  • You are seeking a regenerative alternative before considering radiofrequency ablation or surgical fusion

  • You can commit to a structured recovery protocol

A thorough consultation includes clinical examination with provocative SI joint maneuvers, review of imaging, and confirmation that the SI joint — not the lumbar spine, hip, or piriformis — is the true source of pain.

PROCEDURE​

What does the procedure involve?

The visit takes approximately 60 to 90 minutes:

  1. A small blood draw from your arm, similar to a routine lab visit

  2. Blood is processed in a centrifuge to concentrate platelets

  3. You are positioned on the procedure table

  4. The skin is cleaned and a local anesthetic is applied

  5. Under fluoroscopic (live X-ray) or ultrasound guidance, contrast may be used to confirm correct needle position within the joint

  6. PRP is injected into the joint and, when appropriate, into the dorsal interosseous ligament complex

  7. A small bandage is applied; you go home the same day

Image guidance is essential for SI joint injection. The joint is deep, narrow, and oriented obliquely — blind injection has a poor accuracy rate documented in the literature.

RECOVERY

What is the recovery like?

Days 1-3

Increased soreness at the injection site is expected and often more pronounced than after a steroid injection. Ice and acetaminophen are appropriate. Avoid anti-inflammatory medications (NSAIDs) for two weeks before and six weeks after the procedure — they can interfere with the biological repair process.

Weeks 1 to 2

Activity restriction. Avoid high-impact activities, prolonged sitting, heavy lifting, and asymmetric loading of the pelvis.

Weeks 2 to 6

Gradual return to normal activity. Physical therapy focused on pelvic and core stabilization is often resumed and is an important part of durable recovery.

Weeks 6 to 12

Most patients begin to report meaningful improvement during this window.

3 to 6 months

Peak benefit is typically observed.

Some patients benefit from a second injection 6 to 12 weeks after the first. The decision is made at follow-up based on response.

COMPARISON

How does PRP compare to other SI joint treatments?

  • Physical therapy and stabilization training — first-line management. Important foundation for all other treatments. Should not be skipped.

  • Corticosteroid injection — provides faster pain relief but is anti-inflammatory rather than regenerative. Relief is often weeks to months. Repeated injections have potential effects on cartilage and supporting ligaments.

  • PRP injection — slower onset than corticosteroid but oriented toward durable tissue support rather than short-term pain masking. Often a good middle option for patients who have responded partially to steroid but are seeking longer-term benefit.

  • Radiofrequency ablation (RFA) of the lateral branches — denervates the joint, providing pain relief by interrupting the sensory nerves. Typically lasts 6 to 12 months. Does not address the underlying joint pathology but can be highly effective for pain control. Often considered after PRP if pain persists.

  • Sacroiliac joint fusion — surgical option reserved for refractory cases with confirmed SI joint pathology. Significant procedure with recovery measured in months.

 

       PRP is best positioned between corticosteroid injection and surgical interventions on this spectrum.

PRECISION

The importance of image guidance

The sacroiliac joint is one of the most technically challenging joints in the body to inject correctly. The joint space is narrow, oriented obliquely, and surrounded by ligaments. Studies of blind SI joint injection have shown accuracy rates well below what is acceptable for a procedure intended to deliver a therapeutic into a specific anatomic target.

Fluoroscopic guidance with contrast confirmation, or high-resolution ultrasound, is the standard for SI joint injection in our practice. This is not optional. Without confirmed placement, the injection does not reliably reach the joint and outcomes suffer.

QUALITY

Why preparation matters

Not all PRP is the same. The concentration of platelets, the presence or absence of white blood cells, and the volume delivered all influence outcomes. We use a standardized, validated PRP preparation system that produces a consistent platelet concentration appropriate for spinal and joint applications.

We do not offer PRP derived from minimally processed blood or from off-label kits with limited evidence base.
