What we mean by chronic back pain.

Chronic low back pain is pain that has persisted for more than three months. It is one of the most common reasons adults seek medical care, and it has many possible sources — facet joint arthritis and hypertrophy, sacroiliac joint dysfunction, spinal stenosis, lumbar radiculopathy, disc degeneration, nerve compression, and muscle-related pain, often in combination. The first goal of evaluation is to identify which specific structures are driving your pain, because effective treatment — interventional or regenerative — depends on getting that right.

Our approach to evaluation.

A complete evaluation includes a detailed history - what makes the pain better, what makes it worse, when it started, what has been tried - a focused physical exam that maps the pain to its likely sources, and review of prior imaging. Treatment options.

Treatment is matched to the specific source of pain identified in evaluation. For facet joint pain, image-guided medial branch blocks followed by radiofrequency ablation can produce months to years of relief. For sacroiliac joint pain, image-guided injections and, in selected cases, joint-stabilizing procedures are highly effective. For disc and radicular nerve-related pain, transforaminal epidural steroid injections and selective nerve root blocks are commonly used. For spinal stenosis, image-guided epidural injections are the cornerstone interventional treatment. Throughout, nerve-modulating medications, focused physical therapy, and lifestyle adjustments are layered in — and where appropriate, regenerative options are offered as an alternative to repeated steroid exposure.

Regenerative options.

For select causes of chronic back pain, regenerative treatment with platelet-rich plasma (PRP) is an appropriate option — using your body's own concentrated growth factors to support healing rather than simply masking pain. PRP for facet joint pain and for sacroiliac joint pain has growing evidence and can be especially valuable for patients who want durable relief without the cartilage and ligament concerns of repeated cortisone injections. Every regenerative injection is performed under ultrasound or fluoroscopic guidance for precise placement.
