What is tennis elbow?

Tennis elbow — clinically known as lateral epicondylitis — is a degenerative tendon condition affecting the common extensor tendon at the outside of the elbow. Despite the name, only a small percentage of patients develop it from tennis. It's more often caused by repetitive gripping, lifting, racquet sports, pickleball, weight training, or prolonged keyboard work.

The condition involves microscopic tendon damage (tendinosis), not classic inflammation as the older "-itis" name suggests. This distinction matters: anti-inflammatory medications and corticosteroid injections often fail to provide lasting relief because the underlying problem is degenerative, not inflammatory.

MECHANISM

How does PRP work for tennis elbow?

Platelet-rich plasma is prepared from a small sample of your own blood. The blood is processed in a centrifuge to concentrate platelets, which contain growth factors involved in tissue repair. When injected directly into the damaged extensor tendon under ultrasound guidance, these growth factors are thought to stimulate fibroblast activity, support collagen remodeling, and improve the tendon's structural integrity over time.

PRP is not an immediate pain reliever. The treatment effect develops gradually over several weeks to months as the tendon undergoes biological repair. The goal is durable improvement, not short-term pain masking.

EVIDENCE

What does the research say?

Tennis elbow has one of the strongest evidence bases in the regenerative medicine literature. Multiple randomized controlled trials and systematic reviews have compared PRP to corticosteroid injection — the conventional next-step treatment after failed conservative care.

The general findings across studies:

  • In the short term (under 8 weeks), corticosteroid often provides faster pain relief

  • At 3 to 24 months, PRP demonstrates superior outcomes for pain reduction, function, and grip strength

  • Corticosteroid injections have been associated with higher recurrence rates and potential tendon weakening with repeat use

PRP is not a guarantee. Response rates in published studies typically range from 60 to 80 percent, meaning a meaningful subset of patients do not improve. Honest expectation-setting is part of every consultation.

CANDIDACY

Who is a good candidate?

​​​PRP for tennis elbow may be considered when:

  • Pain has persisted for more than three months

  • Conservative measures have been adequately tried — activity modification, eccentric loading exercises, counterforce bracing, and physical therapy

  • Pain is interfering with daily activities, work, or sports

  • You prefer a regenerative approach over corticosteroid injection or surgery

  • You can commit to a recovery protocol that includes temporary activity restriction

A thoughtful consultation includes clinical examination, review of any imaging (diagnostic ultrasound is often helpful), and a candid discussion of what PRP can and cannot do.

PROCEDURE​

What does the procedure involve?

The visit takes approximately 60 to 90 minutes:

  1. A small blood draw from your arm, similar to a routine lab visit

  2. Blood is processed in a centrifuge to concentrate platelets

  3. The skin is cleaned and a local anesthetic is applied

  4. Under ultrasound guidance, the PRP is injected directly into the affected portion of the extensor tendon

  5. A small bandage is applied; you go home the same day

Ultrasound guidance is standard in our practice for this procedure. Blind injection cannot reliably target the specific area of tendon pathology and is associated with worse outcomes.

RECOVERY

What is the recovery like?

Days 1-3

Soreness and stiffness at the injection site are expected. Ice and acetaminophen are appropriate. Avoid anti-inflammatory medications (NSAIDs) for two weeks before and six weeks after the procedure, as they may interfere with the biological repair process.

Weeks 1 to 2

Temporary activity restriction. Avoid heavy gripping, racquet sports, weight training, and repetitive use of the affected arm.

Weeks 2 to 6

Gradual reintroduction of activity, often guided by physical therapy with emphasis on eccentric loading exercises.

Weeks 6 to 12

Most patients begin to report meaningful improvement during this window.

3 to 6 months

Peak benefit is typically observed.

Some patients require a second injection 6 to 8 weeks after the first if response is partial. This is discussed at follow-up.

COMPARISON

How does PRP compare to other options?

  • Activity modification and counterforce bracing — first-line management for any new presentation. Should be tried before considering injections.

  • Eccentric loading exercises and physical therapy — the foundation of most tennis elbow treatment plans. Often durable when patients adhere to the program.

  • Corticosteroid injection — provides rapid short-term pain relief. Onset within days but often short-lived (weeks to months) with higher recurrence rates. Repeated injections have been associated with potential tendon weakening.

  • PRP injection — slower onset than corticosteroid but better long-term outcomes in randomized trials. Goal is tendon repair support rather than pain masking.

  • Surgical release (Nirschl procedure) — reserved for refractory cases that have failed conservative and injection-based treatments.

Tennis elbow involves a specific anatomic site — the common extensor tendon origin at the lateral epicondyle. The pathologic tissue is often only a few millimeters in dimension. Ultrasound guidance allows real-time visualization of the tendon, accurate identification of the affected fibers, and confirmation that the PRP is delivered exactly where it is needed.

QUALITY

Why preparation matters

Not all PRP is the same. The concentration of platelets, the purity of the preparation, and the consistency from case to case all influence clinical outcomes.
