Testosterone Replacement Therapy (TRT) for Men | Naples & Marco Island
Physician-led testosterone replacement therapy (TRT) for men in Naples & Marco Island — real evaluation of low testosterone, evidence-based treatment, and careful monitoring. It's one part of our broader hormone optimization care at Timeless Interventional of Naples."

OVERVIEW
What low testosterone actually is.
Testosterone is the primary male hormone, and levels naturally decline with age — but a meaningful drop, combined with symptoms, is called hypogonadism (low testosterone). It comes in two forms: primary (the testes aren't producing enough) and secondary (the brain's signaling to the testes is reduced). Distinguishing the two matters, because it changes the workup and sometimes the treatment.
The key principle of good TRT is simple and often ignored: we treat the patient, not the number. A diagnosis requires both genuinely low testosterone on proper lab testing and consistent symptoms — not a single borderline lab in isolation.
SYMPTOMS
Signs of low testosterone.
Low testosterone can show up across your energy, body, mind, and libido — and most men have several at once:
-
Fatigue and low energy, even with adequate sleep
-
Reduced libido and erectile changes
-
Loss of muscle mass and strength; increased body fat, especially around the middle
-
Low mood, irritability, or loss of motivation and drive
-
Brain fog and difficulty concentrating
-
Poor or unrefreshing sleep
-
Reduced sense of well-being — "not feeling like myself"
These symptoms overlap with thyroid problems, sleep apnea, depression, and other conditions — which is exactly why a proper evaluation, not a quick lab, is the right starting point.
WHO IT'S FOR
Is TRT right for you?
TRT is appropriate for men with documented low testosterone (confirmed on proper testing) and consistent symptoms after an appropriate evaluation.
It isn't right for everyone, and part of an honest workup is ruling out other causes and identifying anything that would make treatment inadvisable. If you're considering fertility in the near future, that's an important part of the conversation — see Safety below.
HOW IT WORKS
How a TRT evaluation works.
The consultation begins with a careful history — symptoms, timeline, sleep, medications, and goals — and a focused exam. Laboratory testing is the cornerstone and is done properly: total and free testosterone drawn in the morning (when levels peak), confirmed on a second morning draw before diagnosis, plus LH and FSH (to separate primary from secondary), estradiol, PSA, and a CBC/hematocrit — with additional tests (SHBG, prolactin, thyroid, metabolic panel) as the picture warrants.
From there, the plan is built together — the right formulation and dose for your goals, an expected timeline, and a monitoring schedule. Follow-up labs and symptom check-ins let us adjust to get you the benefit safely. You see the physician directly at every visit.

FORMS & ROUTES
Testosterone therapy options.
Bioidentical hormones can be delivered through several routes. The choice depends on the hormone, the indication, the patient's preferences, and safety considerations.

Injections (testosterone cypionate / enanthate)
-
The most established, cost-effective option
-
Given intramuscular or subcutaneous, typically weekly or every two weeks
-
Steady levels with proper dosing; can often be self-administered at home

Pellets
-
Placed under the skin, release testosterone over several months
-
No daily or weekly routine

Transdermal Gels
-
Applied daily to the skin
-
Steady daily levels; requires care to avoid transfer to partners or children

Fertility-sparing options (hCG, clomiphene/enclomiphene)
-
For men who want to preserve fertility, these stimulate the body's own production instead of (or alongside) TRT — see Safety
SAFETY
Safety & monitoring — done right.
TRT is safe and effective when it's properly monitored, which is where rushed clinics fall short. Our approach:
-
Fertility. This matters and is often glossed over: standard testosterone therapy suppresses your body's own production and can reduce sperm count and fertility. Men who want children now or soon should discuss fertility-preserving options (hCG, clomiphene/enclomiphene) before starting.
-
Red blood cells (hematocrit). Testosterone can thicken the blood; we monitor hematocrit and adjust to keep it in a safe range.
-
Prostate (PSA). We check PSA before and during therapy; TRT isn't used in men with untreated prostate cancer.
-
Estradiol. Some testosterone converts to estrogen; we monitor and manage it when clinically appropriate rather than reflexively.
-
Cardiovascular health. For appropriately selected men, recent high-quality data has been reassuring on cardiovascular safety; we still individualize based on your history.
-
Ongoing monitoring. Lowest effective dose, periodic labs, and symptom check-ins — a plan that's followed over time, not set and forgotten.
The goal is a plan whose benefits clearly outweigh its risks for you — which is exactly what an unhurried, physician-led evaluation is for.
WHAT TO EXPECT
Realistic expectations.
TRT is a process, not an overnight switch. Energy, mood, and libido often improve within the first several weeks; changes in body composition and strength build over months alongside training and nutrition. We're candid about what TRT can and can't do, dial in your dose with follow-up labs, and keep the plan matched to your goals over time.
THE DIFFERENCE
Physician-led, direct-pay care — not a "T mill."
Plenty of clinics will hand any man a prescription off a single lab. That's not us. Because we're a direct-pay practice, you get an unhurried evaluation, a physician who actually reviews your labs and history, and the monitoring that safe therapy requires — with time to get it right.

GOOD TO KNOW
Frequently asked questions
WHAT ARE SYMPTOMS OF LOW TESTOSTERONE?
Common signs include fatigue, low libido and erectile changes, loss of muscle and increased belly fat, low mood or motivation, brain fog, and poor sleep. Because these overlap with other conditions, diagnosis requires both low testosterone on proper morning lab testing and consistent symptoms — not a single number.
DOES TESTOSTERONE THERAPY CAUSE INFERTILITY?
It can. Standard TRT suppresses your body's own testosterone and sperm production, which can reduce fertility. If you want children now or in the near future, tell us up front — fertility-preserving options like hCG or clomiphene can be used instead of, or alongside, therapy.
IS TRT SAFE?
When properly evaluated and monitored, yes. We check and follow hematocrit, PSA, and estradiol, use the lowest effective dose, and individualize based on your history. Recent high-quality data has also been reassuring on cardiovascular safety for appropriately selected men.
HOW SOON DOES TRT START WORKING?
Many men notice better energy, mood, and libido within the first few weeks, while gains in muscle and body composition build over months alongside exercise and nutrition. We adjust your dose with follow-up labs to get the best result safely.
