Peripheral Neuropathy Treatment in Naples & Marco Island

Peripheral neuropathy is one of the most common chronic pain conditions affecting adults, and one of the most undertreated. Whether your neuropathy is related to diabetes, prior chemotherapy, a viral infection like shingles, an autoimmune condition, or an unidentified cause, focused evaluation and a layered treatment plan can substantially improve symptoms and function. What peripheral neuropathy is.

Peripheral neuropathy refers to damage or dysfunction of the peripheral nerves — the nerves that travel from the spine out to the limbs and the body's surface. When these nerves are injured, they can produce a characteristic pattern of symptoms including numbness, burning, electric shock sensations, tingling, allodynia (pain from light touch), and weakness. Many patients also notice balance problems and unsteadiness, particularly in the dark or on uneven surfaces.

Peripheral neuropathy can affect single nerves (mononeuropathy, like carpal tunnel syndrome), multiple separate nerves (mononeuritis multiplex), or many nerves symmetrically (polyneuropathy). The most common pattern is length-dependent polyneuropathy, which affects the longest nerves first — producing the classic "glove and stocking" distribution of symptoms in the feet and hands, with the feet typically affected before the hands.

Common causes.

Diabetes is the leading cause of peripheral neuropathy in adults. Chronic high blood glucose damages small nerve fibers over time, producing the characteristic symmetric, length-dependent pattern. Tight glucose control reduces but does not eliminate the risk.

Chemotherapy-induced peripheral neuropathy (CIPN) is a well-recognized side effect of several common chemotherapy agents, including platinum compounds, taxanes, and vincristine. Symptoms may begin during treatment or shortly after, and can persist for years.

Post-herpetic neuralgia (PHN) is a particularly painful form of peripheral neuropathy that follows shingles. It results from damage to nerve fibers during the acute viral infection and can persist for months or years after the rash resolves.

Other important causes include B12 deficiency (which is treatable and worth identifying), thyroid disease, autoimmune conditions, alcohol-related neuropathy, certain medications, and inherited neuropathies. In roughly 20 to 30 percent of cases, no specific cause is identified — termed idiopathic neuropathy. Even when no cause is found, the condition remains treatable.

How peripheral neuropathy is diagnosed.

Diagnosis combines a detailed history, focused neurologic exam, and selected testing.

The history clarifies the timing of symptoms, the distribution, any associated conditions (diabetes, chemotherapy, prior shingles), and the impact on function. The exam evaluates sensation in the affected areas, deep tendon reflexes, motor strength, and balance.

Laboratory studies look for reversible causes — checking blood glucose and hemoglobin A1c, B12 level, thyroid function, autoimmune markers, and other targeted tests depending on the clinical picture. Identifying and treating reversible causes is a critical first step.

Electrodiagnostic studies (EMG and nerve conduction studies) can characterize the pattern of nerve involvement, distinguish axonal from demyelinating neuropathy, and identify entrapment neuropathies that may be amenable to targeted treatment. In selected cases, additional testing such as skin biopsy for small fiber neuropathy or autonomic testing is appropriate.

Modern treatment of peripheral neuropathy.

Treatment is multi-layered, with several components considered together.

Addressing the underlying cause is the foundation. For diabetic neuropathy, this means tight glucose control. For B12 deficiency, replacement. For autoimmune neuropathies, immunomodulation under the appropriate specialist. Treating the cause reduces ongoing nerve damage even if it does not reverse existing symptoms.

Pharmacologic treatment is the backbone of symptom management. Several classes of medication have established efficacy for neuropathic pain, including gabapentinoids (gabapentin, pregabalin), serotonin-norepinephrine reuptake inhibitors (duloxetine, venlafaxine), and tricyclic antidepressants (amitriptyline, nortriptyline) used in low doses for their nerve-modulating effects. Topical treatments — lidocaine patches and high-dose capsaicin — can be useful for localized neuropathic pain. The right combination is individualized and refined over time.

Interventional treatment is appropriate in specific scenarios. Targeted nerve blocks can identify and treat entrapment neuropathies (for example, carpal tunnel syndrome or meralgia paresthetica). Spinal cord stimulation is well-established for refractory diabetic peripheral neuropathy that has not responded to optimal medical management, and produces substantial improvement in many patients. 

Lifestyle and supportive care matters. Foot care education is critical for diabetic patients to prevent ulceration. Balance training reduces fall risk. Paced movement, sleep restoration, and addressing the autonomic and central nervous system components of pain all support recovery.

Treatment of central sensitization is increasingly recognized as important in patients with longstanding neuropathy, since the nervous system component amplifies pain signaling over time.

Realistic expectations.

Peripheral neuropathy is generally treatable but rarely fully curable, particularly when the underlying nerve damage is established. The realistic goals are meaningful reduction in pain, improved sleep, restored daily function, and prevention of complications such as falls or foot ulceration. Progress is typically measured in weeks to months as treatments are refined.

Patients who understand the multi-mechanism nature of the condition and engage with the layered treatment plan over time tend to have substantially better outcomes than those who hope for a single intervention.
